The important distinguishing factor on ECG is the fact that the ST elevation affects all leads (i.e. is global), as opposed to the localised ST elevation in myocardial infarction. In acute pericarditis, the ECG typically shows ST-segment elevation in all leads, with an upward concavity of the elevation (so-called “smiling face”). The PR segment is depressed.

Pericarditis is classically associated with ECG changes that evolve through four stages. Stage 1 – widespread STE and PR depression with reciprocal changes in aVR (occurs during the first two weeks) Stage 2 – normalisation of ST changes; generalised T wave flattening (1 to 3 weeks) Stage 3 – flattened T waves become inverted (3 to several weeks) Pericarditis, or inflammation of the pericardium, has typical ECG findings. These findings occur in progressive stages, all of which are seen in about 50% of cases of pericarditis. EKG may be diagnostic in acute pericarditis, changes might occur within a few hours of the onset of symptoms, but changes are not always present. Serial electrocardiograms are helpful in patients with acute pericarditis because it causes characteristic 12-lead EKG changes that have typically evolved sequentially through 4 stages 1 2 .
